License Plate Configuration Purchase Process

Buyer submits payment.
Buyer, or the buyer’s giftee, signs and returns Terms of Purchase.
Buyer, or the buyer’s giftee, submits the online form at https://tinyurl.com/cdfcdmvform to Colorado State DMV when ready to register the configuration to a vehicle.
Colorado Disability Funding Committee staff sends a letter to the buyer, or the buyer’s giftee, once the State DMV has updated their internal database.
Buyer, or the buyer’s giftee, takes the letter to their County DMV office to order the physical license plates.
